Job Title: Care Coordinator/LPN

Location: New Orleans, Louisiana

Job Summary:

We are seeking a compassionate and experienced Care Coordinator to join our healthcare team. The ideal candidate will have 2-5 years of experience in a clinical setting and a strong commitment to providing high-quality patient care. This role involves supporting healthcare providers, managing patient records, and ensuring efficient clinical operations.

Key Responsibilities:

  • Patient Care : Assist healthcare providers with patient examinations, treatments, and procedures. Take and record vital signs, medical histories, and patient information.
  • Clinical Support : Prepare examination rooms, sterilize instruments, and ensure all necessary supplies are available.
  • Patient Communication : Educate patients about treatment plans, medications, and follow-up care. Provide emotional support and address patient concerns.
  • Administrative Duties : Schedule appointments, manage patient records, and handle billing and insurance documentation. Ensure accurate and timely entry of patient information into electronic health records (EHR) systems.
  • Compliance and Safety : Adhere to all healthcare regulations and safety protocols. Maintain a clean and safe working environment.
  • Documentation : Maintain accurate and up-to-date patient records, including care plans, progress notes, and any changes in patient status.
  • Crisis Intervention : Respond to patient crises and emergencies, coordinating necessary interventions and support services.
  • Quality Improvement : Participate in quality improvement initiatives to enhance patient care and outcomes.

Qualifications:

  • Education : Licensed Practical Nurse
  • Licensure : Active and unrestricted license in the state of Louisiana (e.g., CMA)
  • Experience : 2-5 years of experience in care coordination, case management, or a related field.
  • Skills :
  • Proficiency in taking vital signs and performing basic clinical procedures.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in electronic health records (EHR) systems.
  • Knowledge of community resources and healthcare systems.

Preferred Qualifications:

  • Experience working with diverse populations.
  • Bilingual abilities are a plus.

Working Conditions:
