Job Description

JOB TITLE: Integrated Health Director

PRIMARY FUNCTION: The Integrated Health Director oversees an expanding Integrated Health Program that includes program development, supervision, consultative services, and direct patient care. The Integrated Health Department includes psychologists, certified substance abuse counselors, tobacco cessation specialists, licensed social workers, licensed clinical social workers, community health workers, psychiatrists, clerks/case managers, trainees and interns, and registered dietitian-nutritionists. The Integrated Health Director provides a mixture of clinical services which include behavioral health consultations, psychiatric assessments, and short-term therapy. Additionally, the Integrated Health Director contributes to organization initiatives such as Trauma-Informed Care, Team-Based Care, and Patient-Centered Medical Home initiatives. The Integrated Health Director supports HR staff professional development and provides staff training on an as needed basis, as well as participates on manager, electronic health record, and quality improvement committees. The Integrated Health Director assumes responsibility for completion of duties with minimal supervision.

POSITION REQUIREMENTS:

  • Able to effectively lead and direct the Integrated Health Department simultaneously to providing direct patient care.
  • Expertise with trauma-informed care and organizational development.
  • Further the department’s development of integrated services.
  • Able to provide high-quality clinical services based on evidence-based practices while completing documentation within 7 days.
  • Functions as a consultant to patients, caregivers, families, and primary care staff.
  • Provides services to patients that are tailored to their stage of readiness for change, effectively provides motivational strategies that lead to improved behavioral, mental, and physical health outcomes.
  • Excellent working knowledge of clinical protocols in an integrated primary care environment serving a broad spectrum of patients and presenting concerns.
  • Competent to participate in team-based care, patient-centered medical home, and population-based care framework.
  • Knowledge and sensitivity to unique diversity issues as they relate to patient health, culture, and socioeconomic circumstances, excellent communication and interpersonal skills.
  • A willingness to learn and demonstrate the principles of trauma-informed care and practice in all aspects of patient care.
  • Strong attunement and adherence to legal and ethical standards of care.

ESSENTIAL DUTIES AND FUNCTIONS

Administrative Duties

  • Oversees and develops Integrated Health programs including primary care behavioral/mental health and psychiatry, substance use, tobacco cessation, and nutrition counseling. Works with other departments on joint/inter-departmental programs (e.g. school-based health, transgender health program, MAT/MOUD, Mommy and Me Tobacco Free).
  • Provides oversight and supervision to Integrated Health Department staff.
  • Completes all personnel management of department staff including hiring, orienting, training, disciplining, and processing paperwork such as timesheets.
  • Assists with writing Integrated Health Department policy, procedures and standards of care.
  • Provides program development to ensure Integrated Health is meeting the mission of the health center and serving the community.
  • Actively participates in organizational initiatives including but not limited to Trauma-Informed Care, Team-Based Care, and Patient-Centered Medical Home.
  • Assists in collecting and preparing data for services provided.
  • Serves as liaison to health plan behavioral health operations (e.g. AlohaCare to improve access to patient care and services).
  • Participates in case conferences with other health care providers and acts as a consultant to staff and patients along with their families.

Clinical Services

  • Assists primary care providers in providing behavioral health consultations to improve healthy behavior and deal with mental health/addiction concerns.
  • Conducts behavioral health consultations, focusing on diagnostic and functional evaluations, then makes recommendations to the primary care provider concerning the patients’ treatment goals and plan.
  • Provides consultation and training to primary care providers to enhance their skill and effectiveness in treating behavioral health needs and increasing patient motivation to make lifestyle changes.
  • Provides brief, focused interventions for patients who need behavioral health services.
  • Advises the primary care provider about which patients are better served at the primary care setting and which should be referred to specialty mental health facilities or elsewhere.
  • Assists in the detection of “at risk” or “high risk” patients and in the development of plans to prevent worsening of their condition.
  • Documents the patient’s progress and diagnostic information in the electronic medical record within 7 days of patient encounter.
  • Helps the patients, where indicated, to cope with chronic conditions like pain and diabetes.
  • Assists the patients in complying with any medical treatment initiated by the primary care provider, such as offering strategies to cope with medication side effects.

OTHER DUTIES

  • Attends and participates in meetings and committees as necessary.
  • Attends additional training sessions as suggested and approved by the CEO to improve deficiencies or enhance knowledge and skills.
  • Performs other duties deemed appropriate and at times selected by the CEO.

Safety and Compliance

  • Attend all required in-service and external training related to Safety and Compliance.
  • Adhere to all policies and procedures in Safety and Health Guidebook.

QUALIFICATIONS

Education : Master’s or doctoral degree in Psychology, Social Work, or a related field; doctoral degree in Psychology preferred.

Experience/Skills :

  • Licensed clinical psychologist or licensed clinical social worker with specialized training in primary care behavioral health or health psychology. At least five years of experience is preferred.
  • Hawaii licensure as a psychologist or clinical social worker required.
  • Experience with Federally Qualified Health Centers or community primary care clinics preferred.
  • Knowledge and experience with trauma-informed care, patient-centered medical home quality measures, and population-based care.
  • Comfort and experience working with patients from different communities across the lifespan. Excellent ability and knowledge to sensitively work with the cultures and lifestyles of the clinic's patient population.
  • Exceptional leadership and management skills as evidenced by training and multiple years of experience. Excellent ability to plan and organize various projects.
  • Excellent communication skills.
  • Valid driver’s license, reliable transportation, and proof of current vehicle liability insurance are required.
  • TB clearance within the past 12 months.
  • BLS CPR, current.

Skills/Competencies:

  • Working knowledge of clinical operations and a full range of patient services across the lifespan.
  • Experience with EMRs (Epic preferred).
  • Microsoft Office (e.g. Work, Excel, Microsoft 365) and Windows skills required.
  • Common database applications processing skills required.
  • Able to communicate effectively with patients, staff, administrators, and others within and outside of the center in written and verbal English; additional languages welcome.
  • Ability to work with multi-ethnic, multi-cultural, and multi-lingual patients and staff.
